Systems Integration of Monroe County, LLC (d/b/a “TogetherNow”) works across a diverse network of committed providers and community members to put people at the center of care in an interconnected system of education, health, and human services. This Privacy Policy is intended to familiarize our participants, providers, partners and others (referred collectively as “you”) with our practices for the gathering, use, disclosure and protection of personal information on our MyWayfinder website and resulting from any interactions with our online or offline offerings related thereto (collectively “Services”).

We take the privacy of your information seriously and encourage you to review this Privacy Policy. If the practices described in this Privacy Policy are not acceptable to you, please do not provide us with information about yourself. By visiting this website, you are agreeing to the practices or policies described below, and you are agreeing to be bound by its Terms of Use in relation to your use of the website.

What Personal Information do we collect from you and how do we use it?

“Personal Information” refers to information that relates to an identified, or identifiable person or household. This includes, for example, your name, home address or email address. We may also collect and process sensitive Personal Information, such as your race and ethnicity information.

We may collect information from a variety of sources in the course of providing Services to you. The Personal Information we collect depends on what Services you use or information you request. Depending on the types of Services sought by you, we may collect the following types of Personal Information:

MyWayfinder Support. If you share feedback or contact us through the MyWayfinder Support Portal, or via email at help@my-wayfinder.org, we may collect information including, but not limited to, your name, telephone number, email address along with any additional information you wish to share. We use this information to help us respond to your inquiry and to contact you regarding our Services.

MyWayfinder Account Creation. If you create an account (“Dashboard”) with us, we may collect the following types of Personal Information from you: your first and last name, home address, email address, password, date of birth, the number and age of persons residing in your household, your marital status, gender, ethnicity, race, emergency contact information. In order to provide Services, we also collect various subjective assessments (“Well-Being Information”) of your satisfaction level with critical life needs (e.g., Food, Health, Housing, Income, Safety, Dependent Care, etc.). All of this information is available on your Dashboard - where you can review, edit or delete such information at any time. We use this information to help identify which service providers might be of interest to you. If you request us to, we will also share the contents of your Dashboard with participating service providers.

Automatic Collection. Our website may automatically collect information about you when you visit. We use this information to help us optimize the design of our website and better suit our visitors’ needs. We use a few different technologies to collect this Information:

Cookies

Our website uses cookies, which are small text files that are stored on the device you use to navigate our website. Cookies may collect information such as the pages your visit on our website, your web address, the type of browser, device or hardware you use, search terms, advertising identifier, mobile network information, and IP-based geography location. To assist with suggesting Providers in your area, we also collect - with your consent - your precise geolocation. • To prevent fraud or potentially illegal activities.

To whom do we disclose your Personal Information?

We may disclose your Personal Information to third parties as specifically requested by you (e.g., if you request to share your MyWayfinder Dashboard with a participating service provider), as may be required by law or as may be required to protect our rights or the safety of our users, or in response to a specific law enforcement request. We may also disclose Personal Information to our affiliates and service providers to accomplish these purposes. We reserve the right to use and disclose de-identified, aggregated data for any legal purpose. Such data does not include any personal information. The purposes may include analyzing and reporting usage trends or community impact results.

Do we sell your Personal Information?

No. We do not sell your Personal Information. Further, we will not process your Personal Information for purposes that are neither reasonably necessary to, nor compatible with, the original purpose for the disclosure without first obtaining your consent.

We require that any third party or service provider receiving your Personal Information sign an agreement maintaining the confidentiality of your Personal Information.

How long do we keep your Personal Information?

Once collected, Personal Information is retained for as long as necessary for the purposes for which the Personal Information was originally collected or when we have an ongoing legitimate business or legal need.

How do we protect your Personal Information?

No system for safeguarding Personal Information or other information is 100% secure. However, we take reasonable efforts to safeguard the security of your Personal Information. Personal Information is stored in a manner consistent with current recognized industry practices, and is protected by administrative, technical, and physical measures to protect the confidentiality, integrity and accessibility of such information and to reduce foreseeable risks of harm arising from such collection, use or retention of Personal Information.

In the event of a breach in the security of our systems, we will adhere to applicable law concerning your rights to be informed of a security compromise to your personal information. New York State’s Stop Hacks and Improve Electronic Data Security Act (SHIELD Act) requires in certain circumstances that we notify individuals impacted by a data breach with further information on the categories of information that were accessed and provide contact information for relevant state and federal agencies that provide information regarding security breach response and identity theft prevention and protection information.

What rights do you have to your Personal Information?

Subject to applicable law and depending on where you reside, you may have the right to request and obtain information about, or copies of, your Personal Information that we process, where we obtained your information, the business or commercial purpose for collecting your information, and the third parties with whom your information is shared. You may also have the right to correct your personal information, or object to its processing. Lastly, you may have the right to ask us for information about when we typically delete stored information or ask that your information not be shared with third parties, or be deleted, anonymized or destroyed, depending on the situation and applicable laws.

We have formalized a procedure detailing the process we follow when receiving such a request. It is important to note that privacy rights are not an absolute. An individual’s fundamental rights must be balanced against legitimate public interest factors. Accordingly, we may refuse to comply with a request; but will clearly explain the reasons in a timely manner.

If you are an authorized agent submitting a request on behalf of another, please submit the request pursuant to the above instructions. We may require documentation to authenticate the resident’s identity and that you are authorized to submit the request on the resident’s behalf in order to be able to fulfill your request. Please keep in mind that if we cannot authenticate that you are authorized to act on the resident’s behalf or authenticate the resident’s identity, we may deny the request.
